Job Title: Social Worker

Location: Newark, DE

Shifts: Evenings or Nights

Sign-on Bonus: $15,000 (with a two-year commitment)

Position Overview:

Pride Health is seeking a compassionate and skilled Social Worker to join our healthcare team in Delaware. This is a full-time position offering evening or night shifts. Key Responsibilities:

• Provide psycho-social support to patients and families in the ED.
• Collaborate with RN Case Managers and Utilization Management to ensure comprehensive care coordination.
• Address and resolve barriers to discharge, supporting the safe transition of patients.
• Assist in establishing and implementing discharge plans.
• Conduct psychosocial assessments to identify patient and family needs.
• Provide counseling, resources, and referrals to help patients navigate healthcare systems.
• Maintain accurate and up-to-date patient records.
• Participate in team meetings and case reviews to ensure the continuity of care.
• Advocate for patient needs while adhering to hospital policies and guidelines.
• Adhere to confidentiality and ethical standards.

Qualifications:

• Education: Master’s degree in Social Work (MSW) or Master of Social Services (MSS) from an accredited graduate school.
• Licensure: LCSW or LMSW certification required by the state of Delaware prior to hire.
• Experience: Acute care experience preferred; experience in emergency departments is a plus.
• Skills: Strong communication, interpersonal, and problem-solving skills.
• Certifications: CCM (Certified Case Manager) preferred, or eligibility to obtain within 18 months.
