LVN or LPN - Home Health Houston, TX Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. We are a group of passionate people dedicated to delivering the highest quality services to our patients, families, team members, and the community. Our team is growing and looking for a dedicated Licensed Vocational Nurse or Licensed Practical Nurse (LVN or LPN) - Home Health Shifts Available: Full-Time & PRN Service Areas with Openings: Northeast / Northwest / Southeast / Woodlands / Clear Lake Responsibilities:

Collaborate with the home healthcare team to develop and implement individualized care plans.
Complete home care nursing visits.
Prepare timely, complete, and accurate clinical notes.
Communicate with healthcare providers and other care team members.
Educate patients and their families about health conditions, treatment plans, and self-care techniques.
Adhere to all agency policies, procedures, and regulatory requirements related to home health care.

Qualifications:

Valid and current LVN or LPN license in the state of Texas or compact state.
Minimum of one (1) year of experience in a home health setting.
Knowledge of home health regulations and general healthcare compliance.
Experience with HCHB EMR. (preferred but not required)
Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
Compassionate and empathetic demeanor with a commitment to providing patient-centered care.
