Home Care Now is hiring LPN's for Medicare/Skilled Visits for PER DIEM positions serving patients in Pinellas county. Must have HOME CARE EXPERIENCE / MUST HAVE MEDICARE KNOWLEDGE performing patient visits in the home. Pay range is $35 - $50/visit. Set your own schedule. EMR System is Axxess. Responsibilities

• Perform patient care following the physician's plan of care and orders
• Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change
• Prepare clinical/progress notes and all required documentation meeting Medicare's standards
• Perform all duties and responsibilities always adhering to Florida chapter 464 - Nurse Practice Act

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing
• Current state license as an LPN in good standing
• Current CPR certification
• Valid driver’s license and reliable transportation
• At least ONE year experience as an LPN at a Medicare Home Health Agency
• Medicare documentation experience required
