Licensed Practical Nurse (LPN) | Intake Coordinator | Home Health in Topeka, Kansas Here at Interim Healthcare, we are committed to delivering high-quality, compassionate home health care services to patients in the comfort of their homes. We are currently seeking an organized, detail-oriented, and empathetic Intake Coordinator to join our Home Health Team to be the first point of contact for patients, families, and referral sources. This LPN Intake Coordinator role comes with some excellent benefits:

As an LPN Intake Coordinator with our Home Health Team, here’s a big-picture view of what you’ll do:
  • Plays a crucial role in the admissions process by handling new referrals
  • Communicate & build relationships with physicians, patients, and families to gather necessary intake information
  • Verifying patient information & insurance
  • Coordinate with clinical and administrative teams
  • Ensuring all documentation is accurate and complete for timely start of care & resumption of care
  • Maintain HIPAA compliance and patient confidentiality at all times
A few must-haves for the LPN Intake Coordinator role:
  • High School Diploma or equivalent required; associate or bachelor's degree preferred
  • Excellent communication, organizational, and multitasking skills
  • Ability to work in fast-paced environment and collaborate with a multidisciplinary team
  • Previous experience in home health, medical intake, scheduling, or healthcare administration
  • Knowledge of Medicare, Medicaid, and Private Insurance guidelines
  • Proficiency with EMR/EHR systems and Microsoft Office Suite
