We're seeking a dedicated Ambulatory Care Clinical Coordinator (Pharmacist) to join our team. What You'll Do:
Elevate Patient Care: Develop and implement quality improvement programs focused on enhancing the pharmacy program and patient outcomes.
Educate and Empower: Create and deliver clinical education programs to improve drug utilization, prescribing patterns, and overall quality of care.
Data-Driven Insights: Analyze pharmacy claim data and generate reports to support program objectives and drive informed decision-making.
Drug Use Review: Conduct retrospective drug use reviews to identify areas for improvement and ensure appropriate medication management.
Lead Quality Initiatives: Design, develop, and implement quality improvement initiatives related to the pharmacy program.
Medication Reviews: Review medication prior authorization requests and appeals, ensuring appropriate access to medications.
Policy and Procedure Enhancement: Review and refine policies and procedures related to pharmacy department functions, including medication therapy management, drug utilization review programs, and prior authorization processes.
Evidence-Based Recommendations: Use an evidence-based approach to conduct new drug reviews, develop formulary recommendations, and establish drug use criteria.
Regulatory Compliance: Assess, review, and respond to federal and state regulatory requirements and audits related to the pharmacy benefit.
Collaboration and Consultation: Consult with clinicians and pharmacists to resolve pharmacy benefit issues and ensure seamless care coordination.
Mentorship and Training: Precept pharmacy students and residents, shaping the future of pharmacy practice.
Care Coordination Focus (if applicable):
Evaluate patient drug therapy regimens and contribute to the development of personalized treatment plans.
Provide medication therapy management (MTM) services to patients.
Utilize technology platforms to document and achieve MTM objectives.
Monitor MTM objectives and high-risk population outcomes.
Conduct medication reviews.
What You'll Need:
Graduate of an accredited pharmacy program.
Current (or eligible) unrestricted pharmacist license.
Advanced pharmacy training (PharmD, residency, fellowship, board certification, or master's degree).
2+ years of clinical pharmacy experience in pharmaceutical care management (ambulatory care or other clinical setting, including residency experience).
What's Even Better (Preferred):
Previous experience in managed care and project management.
PGY1 residency training.
